Position: Letter to DEA PPAEMA Interstate Deployments

IAFC Calls for Federal Clarification on Interstate EMS Operations and Emergency Medications

The International Association of Fire Chiefs (IAFC) is urging the Drug Enforcement Administration (DEA) to clarify federal regulations governing how fire and EMS agencies transport, store and administer controlled medications during interstate emergency responses.

In an October 6, 2026, letter, the IAFC requests additional guidance on the Protecting Patient Access to Emergency Medications Act (PPAEMA) final rule, particularly its application to mutual aid agreements, interstate patient transports, wildfire deployments, Urban Search and Rescue (US&R) operations and federally coordinated disaster responses.

Why This Matters to Fire and EMS Agencies

Fire and EMS personnel frequently respond across state lines to provide lifesaving emergency care. Uncertainty surrounding DEA registration requirements can complicate emergency deployments, resource planning and access to essential medications.

The IAFC is seeking clearer guidance to help departments:

  • Maintain operational readiness during mutual aid and disaster deployments across state boundaries.

  • Ensure regulatory compliance when transporting and administering controlled medications outside their home state.

  • Reduce administrative uncertainty surrounding DEA registration requirements for temporary interstate operations.

  • Support patient safety by maintaining access to critical emergency medications.

  • Strengthen emergency response coordination while maintaining appropriate medication security and accountability.

By advocating for consistent federal guidance, the IAFC aims to help fire and EMS agencies navigate regulatory requirements, maintain operational flexibility and deliver timely, effective emergency medical care when and where communities need it most.

Read the IAFC's letter to the DEA to learn more about the requested clarifications and their potential impact on interstate fire and EMS operations.
